Our outreach aims to educate and assist community members dealing with heirs property and tangled titles—situations where ownership of a home or land is unclear due to inheritance without a will or incomplete legal paperwork. These issues often prevent families from accessing home repair programs, property tax exemptions, or selling or refinancing the property. We provide information on how to establish clear title, connect people with legal resources, and support efforts to preserve generational wealth and housing stability.

I conducted a refresher training for doctors at the Southwest International Clinic on issue spotting and the MLP referral process. The session focused on helping providers identify social determinants of health with potential legal remedies, such as housing instability, benefits denials, immigration issues, and family law concerns. I reviewed how to properly complete and submit referrals, emphasized the importance of timely and detailed documentation, and answered questions about referral trends and case types we commonly handle. The goal was to strengthen collaboration and ensure providers feel confident in connecting patients to our legal services.
